Community Stroke Nurse – Support Patients Post‑Hospital

About the role

Join Chest Heart and Stroke Scotland as a Community Stroke Nurse and help people transition from hospital to home after a stroke. You will provide personalised information, emotional support and practical guidance to empower patients and their families to manage life after stroke.

Key responsibilities

• Facilitate a seamless transition from hospital to community for stroke survivors.
• Deliver personalised advice, information and emotional support to patients and families.
• Enable self‑management by building confidence, skills and knowledge.
• Run the evidence‑based six‑week Chronic Disease Self‑Management Programme.
• Collaborate with multidisciplinary teams to ensure holistic care.

Required profile

• First‑level general nursing qualification.
• Minimum two years post‑registration experience in a hospital or community setting.
• Demonstrated experience working with stroke patients.
• Strong communication, organisational and motivational abilities.

Required skills

• Registered with the Nursing and Midwifery Council (NMC).
• Knowledge of stroke care pathways and self‑management programmes.
