About the role
Whitstable Medical Practice is looking for a qualified or trainee Advanced Clinical Practitioner (ACP) with a passion for frailty and older‑people care to join its General Practice Older Persons (GPOP) team. The role involves supporting care for local care‑home residents, conducting home visits for severely frail patients, and providing urgent assessments to prevent hospital admissions.
Key responsibilities
- Assess frail patients, order investigations and develop management plans.
- Deliver home visits for severe frailty and urgent acute cases.
- Collaborate with GPs, nurses, paramedic practitioners, radiographers and other allied health professionals.
- Provide teaching and support on older‑persons care to colleagues.
- Liaise with community frailty teams and home treatment services.
Required profile
- Special interest in frailty with considerable post‑registration experience.
- Professional registration (e.g., NMC, HCPC).
- Strong organisational skills, flexibility, empathy and compassion.
- Excellent communication, interpersonal and leadership abilities.
Required skills
- Independent prescribing qualification.
- Clinical assessment competence.
- Ability to arrange investigations and interpret results.
