District Nurse Specialist Practitioner – Community Nursing

About the role

An exciting opportunity has arisen within the West Dunbartonshire District Nursing Team for a District Nurse Specialist Practitioner. You will deliver high‑quality, person‑centred care to patients in their homes and communities, working autonomously while leading and supporting the district nursing team.

Key responsibilities

• Lead and manage a defined district nursing caseload, ensuring safe and effective person‑centred care.
• Provide skilled clinical interventions and act as the clinical expert within community nursing practice.
• Coordinate workload, prioritise care needs and respond to both planned and urgent unscheduled requests.
• Work collaboratively with multidisciplinary teams, patients and carers to plan and deliver holistic care.
• Delegate, supervise and support team members to maintain high standards of practice.
• Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards.
• Contribute to service improvement, audit, education and the ongoing development of the district nursing team.

Required profile

• Registered Nurse with current NMC registration in Adult Nursing.
• Specialist Practitioner qualification in District Nursing at Master's level or equivalent.
• V300 Independent Prescribing qualification and advanced clinical assessment skills.
• Demonstrable leadership and management experience in a community or primary‑care setting.
• Evidence of continuous professional development and a commitment to lifelong learning.
• Full driving licence and access to a vehicle.
