Multi-Disciplinary Support Worker

About the role

We are seeking a compassionate and resilient Multi-Disciplinary Support Worker to join our Homelessness Social Care Service in Westminster. You will work with men over 35 who have complex mental health, substance use, and housing needs, helping them progress towards recovery and independence. The role offers a stable 40‑hour week with occasional evening and Sunday shifts.

Key responsibilities

• Build supportive, trusting relationships with customers and external partners.
• Conduct holistic assessments and create personalised support plans.
• Deliver innovative key‑work sessions to achieve support‑plan goals.
• Manage risk using a positive‑risk‑taking approach.
• Facilitate access to health, housing, benefits, and life‑skill services.
• Maintain accurate records and contribute to multi‑agency meetings.

Required profile

• Kind, compassionate and resilient attitude.
• Legally eligible to work in the UK (no sponsorship).
• Ability to work Monday‑Friday 9am‑5pm, one Sunday per month and occasional 12pm‑8pm shifts.
• Experience working with adults with mental health or complex needs is desirable.
