Social Worker – Transitions Team

About the role

The Transitions Team is seeking a qualified Social Worker to support young people aged 17‑25 as they move into adulthood. This role offers the flexibility of locum work while delivering person‑centred interventions that promote independence and resilience.

Key responsibilities

• Manage a caseload of 17‑25‑year‑olds, conducting Care Act assessments, reviews and support plans.
• Apply the 3 Conversations Model to deliver strengths‑based interventions.
• Carry out mental capacity assessments and contribute to Best Interest decisions under the Mental Capacity Act.
• Develop and commission care packages, including personal budgets and direct payments.
• Lead and attend multi‑agency meetings such as EHCP reviews, Section 117 after‑care planning and Continuing Healthcare processes.

Required profile

• Degree or equivalent in Social Work (BA/BSc/MA, DipSW or CQSW).
• Registered with Social Work England.
• Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005 and Equality Act 2010.
• Understanding of Safeguarding Adults procedures and statutory guidance.
• Familiarity with the Preparing for Adulthood framework, SEND processes and related legislation.
