About the role
Change Grow Live is seeking a qualified pharmacist to join its multi‑disciplinary team as a Non‑Medical Prescriber (NMP). The NMP will lead the delivery of Hostel In Reach clinics, providing assessment, prescribing, and ongoing medication management for clients with alcohol and drug dependencies.
Key responsibilities
- Assess, diagnose, plan, implement and evaluate treatment and interventions for service users.
- Prescribe and review opiate and alcohol substitute medications in line with evidence‑based practice and local protocols.
- Provide clinical advice to colleagues and partner agencies on prescribing matters.
- Update and refer service users for BBV vaccination and immunisation programmes.
- Promote health‑promotion strategies and support clients in adopting healthier lifestyles.
- Maintain accurate, contemporaneous records of all consultations.
- Participate in self‑review, peer review and formal evaluation of care delivery.
Required profile
- Qualified pharmacist with current GPhC registration.
- Minimum two years post‑qualification experience in a relevant clinical setting.
- Ability to work flexibly, listen actively and respond to urgent or emergency situations.
- Strong communication skills with service users, carers and external stakeholders.
