About the role
We are seeking two experienced Adult Social Workers to join a dedicated Transitions team. The role is primarily remote, with one day per week required in the office, supporting adults aged 18‑25 with complex needs.
Key responsibilities
- Provide social work support to adults in transition, focusing on disability and complex needs.
- Collaborate with multidisciplinary teams to develop and implement care plans.
- Maintain accurate case records and ensure compliance with regulatory standards.
- Participate in weekly office day for team meetings and supervision.
Required profile
- Registered with Social Work England.
- Degree in Social Work.
- Experience working in a Transitions team or with adult disabilities.

What we offer
- Hybrid working model – remote with one office day per week.
- Competitive rate of £40 per hour minimum.
- 37‑hour work week on an ongoing contract basis.
