Clinical Pharmacist – Hospital Pharmacy

About the role

The Alexandra Hospital, part of Circle Health Group, is seeking a Clinical Pharmacist to join its Pharmacy Department. This full‑time position works 37.5 hours per week on a flexible shift pattern, providing essential medication expertise to in‑patients and out‑patients.

Key responsibilities

• Conduct medication reviews for in‑patients and out‑patients, identifying interactions, contraindications and optimisation opportunities.
• Provide tailored advice on treatment plans, side‑effects, compliance and lifestyle considerations.
• Support safe prescribing through medication reconciliation, clinical screening and dose adjustment.
• Review and authorise prescriptions in line with clinical guidelines, formulary restrictions and regulatory requirements.
• Collaborate with procurement, stock control and pharmacy technicians to ensure medicine availability and supply continuity.
• Monitor and report medication incidents, near misses and adverse drug reactions according to hospital protocols and MHRA guidance.

Required profile

• A degree in pharmacy (MPharm, BSc or BPharm) and GPhC registration.
• Previous experience in a clinical hospital pharmacy role.
• Strong knowledge of therapeutics, clinical guidelines and pharmaceutical legislation.
• Excellent communication and clinical decision‑making skills.
• Ability to work autonomously and as part of a multidisciplinary team.

Required skills

• Proficiency with electronic prescribing and medication administration systems (e.g., EPMA).
• Medication reconciliation and dose‑adjustment expertise.
