Discharge Support Worker – Mental Health (Part‑time)

About the role

We are looking for a Discharge Support Worker to join our Mental Health Floating Support team delivering in‑reach services on several hospital wards across Greater Manchester. The role is part‑time, up to 32.5 hours per week, and focuses on removing barriers to discharge such as homelessness, financial difficulties or a history of offences.

Key responsibilities

• Carry out person‑centred assessments to identify housing and support needs.
• Set clear targets, timescales and quality standards for each case.
• Provide practical assistance in accessing housing, financial advice and other support services.
• Build and maintain links with housing providers, social services and community organisations.
• Ensure smooth transitions from hospital to community living.

Required profile

• Experience supporting individuals with mental health needs.
• Understanding of housing, financial and social inclusion issues.
• Ability to work independently and as part of a multidisciplinary team.
• Eligibility to work in the UK and reside in the area.

Required skills

• Person‑centred assessment and planning.
• Knowledge of local housing and support pathways.
• Effective communication with service users and external agencies.
