Clinical Practitioner – Functional Conditions (Managed Service Network)

About the role

The Clinical Practitioner will join a newly funded Managed Service Network (MSN) in NHS Grampian, supporting adults with functional conditions. This secondment position focuses on delivering patient‑centred, goal‑directed therapy and promoting self‑management approaches.

Key responsibilities

• Provide clinical assessment, collaborative formulation and practical support to individuals.
• Deliver individual appointments and guided self‑management conversations.
• Develop and implement care pathways, education materials and self‑management resources.
• Work within a small multidisciplinary team hosted in Surgery 3 of the Acute Sector.
• Participate in publicity activities to showcase the impact of the grant‑funded project.

Required profile

• Clinical background from any discipline, with a passion for person‑centred care.
• Vision, drive and enthusiasm to develop new services for functional conditions.
• Willingness to engage in project publicity and collaborate across the health system.
• Agreement from current line manager if applying for the secondment.
