Community Reablement Assistant

About the role

The Community Reablement Assistant joins the North Lincolnshire Home First Community Rehabilitation service to support individuals in their own homes after hospital discharge, accidents, or illness. Working within a fast‑moving transitions team, you will help people regain independence and maintain safety in the community.

Key responsibilities

• Deliver time‑limited reablement and rehabilitation support following therapy plans.
• Provide direct care such as moving and handling, personal care, and medication administration.
• Respond to crises, including falls or carer breakdowns, to ensure rapid assistance.
• Support social inclusion by sign‑posting users to external organisations and professionals.
• Collaborate with health and social care professionals to coordinate care plans.

Required profile

• Commitment to person‑centred care and promoting independence.
• Willingness to work a rotating roster of 3‑4 shifts per week between 07:00 and 23:00.
• Desire for a 25‑hour per week contract with opportunity for overtime.
• Previous care qualification desirable but not essential; full training provided.

Required skills

• Basic knowledge of moving and handling techniques.
• Understanding of medication administration procedures.
• Ability to follow written therapy and care plans.
