As a Housing Support Coordinator, you will play a pivotal role in empowering adults to secure and maintain stable housing through person-centered support and skill development.

Missions principales

• Provide comprehensive support packages to help individuals secure and sustain independent living.
• Manage a dedicated caseload, conducting thorough needs and risk assessments.
• Develop, implement, and regularly review personalized support plans.
• Perform outreach work directly in clients' homes to ensure continuous support.
• Provide emergency/on-call service coverage for the reception service.
• Collaborate effectively with partner agencies and stakeholders to promote well-being and safeguarding.

Profil recherché

• A passionate individual dedicated to supporting vulnerable populations.
• Possess transferable skills from the social care sector.
• Hold (or be willing to work towards) an NVQ Level 3 in Health and Social Care.
• Strong understanding of housing options and relevant legislation.
• Deep understanding of safeguarding principles and protection protocols.
• Must hold a full UK driving license.

Competences requises

• Excellent communication, organizational, and time management skills.
• Proficiency in basic IT tools and Microsoft Teams.
• Ability to manage complex caseloads and risk assessments.
