Social Care Practitioner

About the role

We are seeking a Social Care Practitioner to join our multidisciplinary Transitions Hub in Bristol. The role supports young people aged 14‑25 as they move from children’s services to adult services, helping them develop independent living skills and access appropriate care.

Key responsibilities

• Conduct Care Act assessments, support conversations, carer assessments and mental capacity assessments for young people and their families.
• Develop and review support plans and commissioning recommendations to secure needed services, equipment and community resources.
• Work collaboratively with social workers, occupational therapists, health, education, housing and police partners to ensure seamless transition pathways.
• Provide early‑help interventions within the Transitions Hub and promote equality and inclusion throughout service delivery.
• Travel across Bristol and surrounding areas to meet service users as required.

Required profile

• Qualified to work within the statutory framework of the Care Act 2014.
• Experience delivering assessments and support planning for young adults with additional needs.
• Strong commitment to person‑centred practice and a whole‑family approach.
• Ability to build effective partnerships with a wide range of external agencies.

Required skills

• Knowledge of Care Act assessments, support conversations, carer assessments, mental capacity and best‑interest assessments.
• Understanding of equality and inclusion principles in social care.
