Adult Community Social Worker

About the role

We are looking for a dedicated Adult Community Social Worker to join our team in Warrington. You will provide assessment, support planning and care management for adults living in the community, promoting independence and choice in line with the Care Act 2014.

Key responsibilities

• Conduct person‑centred Care Act assessments for adults with physical, learning, mental health or age‑related needs.
• Develop, implement and review complex support plans that deliver value for money and promote well‑being.
• Lead safeguarding enquiries, manage complex adult safeguarding cases and contribute to strategy meetings.
• Carry out comprehensive risk assessments and create robust risk‑management plans.
• Assess mental capacity and document decisions according to the Mental Capacity Act 2005 and Best Interests principles.
• Collaborate with health, housing, voluntary and council partners to deliver integrated support.
• Maintain accurate, timely records on the electronic case management system, ensuring statutory compliance.

Required profile

• Qualified Social Worker (QSW) with current registration with Social Work England.
• Recent experience in a statutory adult social‑care setting (community, locality or integrated team).
• Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and local safeguarding procedures.
• Excellent communication, assessment, risk‑management and organisational abilities.
