Adult Social Care Transformation Lead

About the role

The Adult Social Care Transformation Lead will guide a six‑month organisational change programme for a local authority in London. Working in a hybrid model, you will partner with health, housing, voluntary and community organisations to embed a neighbourhood‑based approach that enhances outcomes for service users.

Key responsibilities

• Provide strategic leadership for the transition to a neighbourhood‑based model of adult social care.
• Embed strengths‑based, person‑centred and outcome‑focused practice across teams.
• Coach, mentor and develop managers and practitioners to build professional confidence and leadership.
• Strengthen partnerships with health, housing, voluntary and community sectors to support co‑production and community asset integration.
• Oversee delivery of statutory Care Act duties, ensuring quality, performance and risk management.
• Promote independence, prevention and positive risk‑taking in service delivery.
• Support effective deployment of financial and workforce resources.

Required profile

• Senior leader with recognised professional registration (e.g., Social Work, Occupational Therapy or another Allied Health Profession).
• Extensive knowledge of the Care Act 2014, including wellbeing, prevention and safeguarding.
• Strong understanding of strengths‑based practice and neighbourhood models.
• Proven experience of leading culture change, service transformation and practice improvement.
• Experience of hospital discharge social work is essential for at least one assignment.
