Adults Advanced Practitioner – Front Door Services

About the role

The Adults Advanced Practitioner will join Salford Council’s Front Door services, providing professional leadership and high‑quality, strengths‑based practice across Early Help and Safeguarding. You will conduct complex assessments, support safeguarding enquiries and work closely with health and social care partners to deliver person‑centred interventions.

Key responsibilities

• Undertake complex Care Act assessments, reviews and safeguarding enquiries.
• Deliver strengths‑based, person‑centred interventions that promote independence and wellbeing.
• Provide professional advice, guidance and mentoring to less experienced staff.
• Conduct supervision sessions with allocated practitioners.
• Perform case audits and sign‑off responsibilities to ensure safe decision‑making.
• Support quality assurance activities and service‑improvement initiatives.
• Assist the Team Manager with day‑to‑day operations, work prioritisation and duty functions.
• Promote preventative approaches, early intervention and integrated working across health and social care.

Required profile

• Qualified Adult Social Work practitioner with Advanced Practitioner experience.
• Ability to lead, mentor and support professional development of team members.
• Willingness to register with Remedy for locum positions.
• Successful completion of an Enhanced DBS disclosure.
• Commitment to safeguarding and statutory responsibilities.
