Adults Advanced Practitioner – Urgent Care

About the role

Our client, a Greater Manchester Council, is looking for an Adults Advanced Practitioner to join its Urgent Care Team in Old Trafford. The role offers a competitive rate of £32 per hour and provides an opportunity to lead high‑risk, complex adult social care cases.

Key responsibilities

• Lead and oversee complex, high‑risk urgent adult social care cases.
• Conduct advanced assessments, risk assessments and develop care plans for adults with complex needs.
• Support crisis intervention, hospital avoidance and safe discharge planning.
• Provide coaching, mentoring and practice guidance to social workers and newly qualified staff.
• Participate in and chair multi‑disciplinary case discussions and professional meetings.
• Collaborate with health, community, housing and partner agencies to coordinate urgent interventions.
• Apply expertise in the Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
• Contribute to quality assurance, case audits and continuous service improvement.

Required profile

• Qualified Social Worker registered with Social Work England.
• Minimum three years’ experience as a qualified social worker.
• Demonstrated ability to manage complex, high‑risk cases.
• Strong knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
