Adults Social Worker – Integrated Support Team

About the role

Our client, Wirral MBC, is seeking an Adults Social Worker to join an integrated and supportive team. The role focuses on delivering high‑quality social work support to adults, their families and carers while preventing unnecessary hospital admissions.

Key responsibilities

• Carry out needs‑led assessments, complex case work, care and support planning, and reviews.
• Initiate and contribute to multi‑disciplinary assessments and meetings.
• Provide safeguarding enquiries for adults at risk of abuse or neglect in line with the Care Act 2014.
• Communicate assessment information, risk factors and care plans to stakeholders using appropriate ICT tools and information‑governance standards.
• Participate in community forums and promote positive social work practice, especially for older adults and those with physical or mental health needs.
• Chair multi‑disciplinary and strategy meetings, ensuring all relevant documentation is collated and available.

Required profile

• Qualified adult social worker with experience in complex assessments.
• Knowledge of the Care Act 2014 and adult safeguarding policies.
• Ability to work effectively within a multi‑disciplinary team.
• Strong communication skills for conveying sensitive information.
