Adults Social Worker – Physical Disability & Sensory Impairment

About the role
The Council is seeking an Adults Social Worker to join the Physical Disability & Sensory Impairment team in Caerphilly. You will work directly with service users, families and partner agencies to assess needs, manage risk and deliver tailored support.
Key responsibilities
- Carry out assessments, reviews and care planning in line with National Occupational Standards and the Code of Practice for Social Care Workers.
- Maintain accurate records of outcomes, risks and needs for each service user.
- Collaborate with multidisciplinary teams, including health trusts and other social services, during normal hours, evenings and weekends.
- Develop and improve services, ensuring integration across health and social care.
- Respond to crisis situations and support individuals in making informed decisions.
Required profile
- Professional competence in adult social work practice.
- Ability to manage and be accountable for your own practice with supervision.
- Strong understanding of risk assessment and health & safety responsibilities.
- Commitment to advocacy and promoting independence for service users.
