Associate Case Manager (Self‑Employed) – Neurological Rehabilitation

About the role

This self‑employed position combines specialist neurological occupational therapy with case management to support adults and children recovering from complex injuries. You will design and coordinate personalised rehabilitation programmes, working remotely and travelling to client sites as needed.

Key responsibilities

• Assess client needs and develop person‑centred rehabilitation plans with measurable goals.
• Coordinate multidisciplinary teams to deliver and monitor effective rehabilitation interventions.
• Conduct risk assessments, manage budgets and ensure cost‑effective service delivery.
• Recruit, supervise and support care staff, identifying training requirements.
• Produce accurate clinical reports, maintain compliant records and liaise with legal representatives, healthcare professionals and external agencies.
• Provide specialist occupational therapy interventions within your professional scope where appropriate.

Required profile

• HCPC registration and a recognised occupational therapy qualification.
• Experience in neurological rehabilitation and complex clinical assessment.
• Knowledge of case‑management standards, clinical governance and multidisciplinary rehabilitation.
• Ability to produce high‑quality clinical documentation and manage a complex caseload.
• Full UK driving licence, access to a vehicle and willingness to travel across client locations.
