Band 5 Ward Nurse – Sapphire Ward

About the role

As a Band 5 Ward Nurse you will join the Neuro‑Stroke Team on Sapphire Ward at Gravesham Community Hospital. The 21‑bed specialist unit provides neurological rehabilitation for patients recovering from stroke and other neurological conditions, helping them regain independence.

Key responsibilities

• Assess, plan, deliver and evaluate nursing care for patients with stroke and a range of neurological disorders.
• Collaborate with the multidisciplinary team to implement personalised rehabilitation programmes.
• Support patients and families throughout the recovery journey, promoting independence and quality of life.
• Maintain accurate patient records and ensure compliance with NHS policies and safety standards.

Required profile

• Registered Nurse eligible for Band 5 placement.
• Willing to work full‑time (37.5 hours per week) on an onsite basis.
• Interest in neuro‑rehabilitation, either newly qualified or seeking to develop clinical skills.
