Band 6 Occupational Therapist

About the role
This full‑time Band 6 Occupational Therapist role is based in Brentwood, supporting stroke patients throughout the Mid‑Essex area. You will act as a key worker, delivering evidence‑based, person‑centred therapy within a multidisciplinary team.
Key responsibilities
- Manage a caseload of stroke patients, conducting comprehensive OT assessments.
- Set person‑centred goals and develop treatment plans in collaboration with patients and carers.
- Monitor, evaluate and adapt interventions within a multidisciplinary framework.
- Coordinate discharge planning and act as the patient’s key worker.
- Collaborate with the stroke rehabilitation team, reablement services and voluntary sector for post‑discharge support.
- Maintain accurate client records on IT databases in line with trust policies.
- Stay up‑to‑date with HCPC standards, NICE guidelines and relevant clinical research.
Required profile
- Registered with HCPC and able to meet its proficiency standards.
- Strong knowledge of neuro‑anatomy, stroke presentations and their impact on cognition and function.
- Experience working within multidisciplinary teams and with external agencies.
- Ability to handle moving and handling tasks safely in community settings.
Required skills
- Proficiency with IT database systems for client record management.
