Brokerage Officer – Care Navigation Team

About the role

We are seeking a Brokerage Officer to join the Care Navigation Team in Preston. The role supports adults and young people transitioning to adult services across Lancashire, delivering person‑centred care solutions.

Key responsibilities

• Arrange personalised, cost‑effective care and support packages.
• Co‑produce support plans with individuals, families and professionals.
• Source services through approved providers, framework agreements and community resources.
• Identify free or low‑cost services that promote independence.
• Negotiate placements and care packages to ensure value for money and meet assessed needs.

Required profile

• Experience in health, social care, brokerage, commissioning or a related field.
• Strong organisational and problem‑solving abilities, able to manage competing priorities.
• Excellent communication and negotiation skills.
• Person‑centred approach with commitment to independence and choice.
• Ability to build effective relationships with a range of stakeholders.
• Confidence working with budgets, providers and care packages.
• Capability to work independently and as part of a collaborative team.
