Care Coordinator

About the role

The Care Coordinator role at Apollo Home Healthcare supports clients in Heywood and surrounding areas, ensuring safe, dignified care in their homes. You will act as the main point of contact for families and professionals, coordinating schedules and advocating for client needs.

Key responsibilities

• Serve as the first point of contact for clients, families, and external professionals.
• Manage rotas, make rapid scheduling decisions and match HCAs and nurses to care packages.
• Monitor care delivery to ensure compliance with care plans and quality standards.
• Attend multi‑agency ICB meetings and represent client interests.
• Support the out‑of‑hours on‑call service (non‑hands‑on).

Required profile

• Proven experience as a Care Coordinator.
• Strong scheduling and organisational abilities.
• Clear, adaptable communication and calm, solution‑focused decision‑making.
• Understanding of person‑centred care and care planning.
• Confident negotiation, influencing skills and ability to build strong relationships.
