Care Team Leader

About the role

We are seeking a dedicated Team Leader to join the Adult Social Care and Health Department’s Reablement Community Team in Lewes, East Sussex. The role leads support workers delivering day services for older people and community support for adults with learning disabilities, helping clients live independently and achieve their personal goals.

Key responsibilities

• Lead, supervise and motivate a team of support workers across day services and community settings.
• Plan, coordinate and deliver person‑centred activities and care plans that align with client goals.
• Conduct monthly quality checks, maintain accurate records and ensure compliance with health, safety and CQC standards.
• Manage induction, training and ongoing professional development for team members.
• Participate in detailed client support planning, risk assessments and act as premises controller when required.

Required profile

• Lead Adult Care Worker Level 3 Apprenticeship or equivalent, or willingness to complete.
• Level 2 Maths and English or ability to pass assessment.
• Proven experience leading teams in a care setting, preferably with older adults or learning disabilities.
• Excellent organisational, communication and interpersonal abilities.
• Awareness of CQC standards and safeguarding vulnerable adults.
