Care Team Leader – Supported Living Service

About the role

Glenholme Healthcare is seeking an experienced Care Team Leader to manage its specialist Supported Living Service at Vincent Place, Stevenage. You will be responsible for guiding a team of Support Workers to deliver high‑quality, person‑centred care for individuals with learning disabilities, autism, physical disabilities and complex needs.

Key responsibilities

• Lead, motivate and support a team of Support Workers to ensure exceptional care delivery.
• Develop and oversee tailored care plans in collaboration with healthcare professionals.
• Maintain compliance with CQC standards and drive continuous quality improvement.
• Build positive relationships with residents, families and external partners.
• Provide hands‑on support, including personal care, medication administration and daily living tasks.

Required profile

• Previous leadership experience in health or social care, preferably within learning disability services.
• Strong communication and interpersonal abilities to inspire and guide staff.
• A compassionate, person‑centred approach with a commitment to dignity and respect.
• Solid understanding of CQC standards and relevant legislation.
• Flexibility to work a range of shifts, including evenings, weekends and bank holidays.
