Children's Community Nurse

About the role

As a Band 6 Community Children’s Nurse you will provide high‑quality nursing care to children and young people aged 0‑18 across West Essex. Working from Harlow, you will support families in their homes and community settings, ensuring safe and effective care for acute, chronic and complex health needs.

Key responsibilities

• Deliver specialist community‑based nursing care to children and young people.
• Assess, plan, implement and evaluate individualized care plans with families.
• Support early discharge from hospital and coordinate ongoing community care.
• Provide palliative and end‑of‑life care where required.
• Collaborate with hospital teams, GP practices and other community professionals.

Required profile

• Registered Nurse qualified to work with children and young people.
• Experience in community nursing or paediatric care.
• Ability to work within a multidisciplinary team and communicate effectively with families.

Required skills

• Clinical assessment and care planning for paediatric patients.
• Medication management and administration.
• Knowledge of palliative and end‑of‑life care principles.
