Clinical Case Manager – Flexible Part‑time Remote

About the role

Bush and Company Rehabilitation is expanding its network of clinicians to provide flexible, home‑based case management for adults with complex rehabilitation needs. As a Clinical Case Manager you will work remotely, delivering high‑quality, evidence‑based rehabilitation planning and coordination for individuals recovering from life‑changing injuries.

Key responsibilities

• Conduct assessments in the client’s home or community setting.
• Develop, implement and regularly review personalised rehabilitation recommendations and plans.
• Coordinate appropriate rehabilitation services and external providers.
• Support goal‑setting and outcome‑focused rehabilitation planning.
• Liaise with multidisciplinary professionals as required for each case.
• Produce clear, professional case documentation and reports.
• Facilitate rehabilitation pathways and ensure safe, effective environments.

Required profile

• Qualified clinician (Physiotherapist, Occupational Therapist, Social Worker, Sports Science Professional or Registered General Nurse) with extensive experience in rehabilitation after serious injury.
• Strong understanding of the impact of catastrophic injuries on individuals and families.
• Proactive, organised, solution‑focused with excellent clinical reasoning and communication skills.
• Ability to build trusted relationships with clients, families and multidisciplinary teams.
• Comfortable working independently and managing own time within contractual requirements.
