Community Health Navigator – Full‑Time

About the role

The Community Health Navigator is the first point of contact for patients, the public and health professionals accessing the Bath & North East Somerset, Swindon and Wiltshire Community Health Services Single Point of Access. You will guide callers and email enquiries, ensure referrals are processed correctly and help patients reach the right service quickly.

Key responsibilities

• Answer inbound calls, emails and post with empathy, assess urgency and direct enquiries to the appropriate service.
• Maintain accurate patient records and update key IT systems, including processing referrals and correspondence.
• Book, cancel and reschedule clinical appointments, meeting waiting‑time targets.
• Identify and escalate incomplete or urgent referrals to clinical colleagues.
• Provide administrative support to clinicians and managers to keep services running smoothly.
• Work flexibly within a supportive team, contributing to service priorities as needed.
