Community Mental Health Support Worker

About the role

This permanent position is based at Assynt Medical Practice in Lochinver, supporting individuals experiencing mental health difficulties in their own homes and local communities across West Sutherland. You will work closely with Community Mental Health Nurses and a multidisciplinary team to deliver person‑centred, recovery‑focused care.

Key responsibilities

• Provide practical and emotional support to service users in their homes and community settings.
• Assist with the development and implementation of care plans, promoting self‑management and early intervention.
• Collaborate with nurses and other professionals to ensure coordinated, recovery‑focused care.
• Help prevent deterioration and reduce the need for more intensive services.
• Maintain accurate records and communicate effectively with the wider team.

Required profile

• Strong communication skills and a compassionate, empathetic approach.
• Ability to work both independently and as part of a multidisciplinary team.
• Full driving licence; access to a vehicle for work purposes may be required.
• Motivation to support individuals in a community setting.
