Community Nurse – Community Nursing

About the role

The Community Nurse will join NHS Greater Glasgow and Clyde’s community nursing team, delivering high‑quality care to patients in their own homes and community settings. Working closely with the District Nurse, you will manage a caseload of patients, develop personalised care plans and ensure safe, evidence‑based practice.

Key responsibilities

• Assess, plan, implement and evaluate individual care programmes for community patients.
• Maintain and update patient records in line with local, regional and national policies.
• Collaborate with patients, families, carers and multidisciplinary teams to deliver holistic care.
• Provide cover for the caseload holder during sickness, absence or annual leave.
• Support and supervise health support workers and junior nursing staff as required.
• Participate in out‑of‑hours and on‑call duties, including public holidays.

Required profile

• Current registration with the Nursing and Midwifery Council (NMC) and a valid PIN.
• Membership of Disclosure Scotland’s PVG scheme.
• Minimum two years (24 months) of community nursing experience.
• Ability to work autonomously and make clinical decisions.
• Strong communication skills and cultural competence.

Required skills

• NMC registration and PVG clearance.
• Experience in patient assessment and care planning.
• Knowledge of NHS policies, procedures and guidelines.
