Community Nurse – New Scots Coordinator (Fixed‑term)

About the role

This position is for a passionate registered nurse to act as a New Scots Community Nursing Coordinator. You will deliver high‑quality, person‑centred care to refugees, asylum seekers and other newly arrived residents across Glasgow and surrounding communities, helping them access health services and improve wellbeing.

Key responsibilities

• Coordinate and manage a caseload of New Scots patients with complex health and social care needs.
• Conduct comprehensive assessments, develop care plans and evaluate evidence‑based nursing interventions.
• Provide clinical leadership and guidance to nursing staff, healthcare support workers and multidisciplinary colleagues.
• Collaborate with health, social care, third‑sector and partner agencies to ensure effective care coordination and discharge planning.
• Support patients in accessing appropriate healthcare services while promoting health improvement, wellbeing and independence.
• Lead clinical risk assessment, incident reporting, quality improvement activities and implementation of best‑practice standards.
• Contribute to staff development through mentorship, training, supervision and support for team members.

Required profile

• Registered Nurse with a current NMC first‑level registration.
• Evidence of continuing professional development and commitment to lifelong learning.
• Strong communication and interpersonal skills with the ability to engage effectively with diverse communities and partner organisations.
• Demonstrable leadership, organisational and case‑management abilities.
