Community Rehabilitation Specialist Occupational Therapist

About the role

This permanent Band 6 position offers an HCPC‑registered Occupational Therapist the chance to lead a complex caseload of older and disabled adults across the North West of Glasgow. Working from Maryhill Health Centre, you will deliver person‑centred assessment, treatment and advice in community settings, including patients' homes and Discharge to Assess units.

Key responsibilities

• Conduct comprehensive assessments and develop individualized care plans.
• Provide therapeutic interventions to promote independence and self‑management.
• Coordinate cases with multidisciplinary teams and support timely hospital discharge.
• Cover duties, supervise staff and students, and contribute to health‑promotion activities.
• Participate in audit, service development and quality improvement projects.

Required profile

• HCPC registration and Band 6 NHS grade.
• Full UK driving licence for travel across the service area.
• Experience in community practice, older‑people care or neurological conditions (desirable).
• Ability to work autonomously and as part of a dynamic multidisciplinary team.
