Community Staff Nurse – District Nursing (12‑month Fixed Term)

About the role

The District Nursing Service delivers case management and skilled clinical interventions across community settings. As a Community Staff Nurse you will work alongside a District Nurse to support patients in their homes or residential care facilities, helping them maintain independence and avoid unnecessary hospital admissions.

Key responsibilities

• Assist the District Nurse with clinical duties, including long‑term condition management, wound care and end‑of‑life care.
• Promote patient independence and support safe, timely discharge from hospital.
• Provide health education and health promotion to patients and their carers.
• Deliver care in patients’ homes and residential care settings, covering a flexible schedule that includes weekends and back‑shifts.

Required profile

• Previous NHS experience is desirable but not essential.
• Full UK driving licence is desirable due to the community‑based nature of the role.
• Effective communication and organisational skills.
• Compassionate, person‑centred approach to patient care.
• Ability to work independently and as part of a multidisciplinary team.
• Knowledge of delivering care within community and residential settings is desirable.
