Community Staff Nurse – District Nursing

About the role

This permanent, full‑time Band 5 position offers an experienced Registered Nurse the chance to deliver high‑quality community care across the Vale Centre for Health & Care. You will work autonomously, providing person‑centred nursing to patients in their homes or local health‑centre settings while being supported by a multidisciplinary team.

Key responsibilities

• Assess, plan and implement care programmes in partnership with patients, families and carers.
• Temporarily manage caseloads during colleague absence, seeking senior support when required.
• Use clinical and environmental risk‑assessment tools to identify hazards and apply appropriate interventions.
• Report and review incidents to prevent recurrence.
• Support patients, families and carers to achieve optimal health and independence.
• Prioritise workload, delegating tasks appropriately to meet patient needs.
• Provide care in treatment rooms or health‑centre settings as needed.
• Participate in clinical audit, research and evidence‑based practice improvement.
• Adhere to NHSGGC policies, NMC Code and national guidelines.
• Act as a clinical leader and positive role model for junior staff.

Required profile

• Registered Nurse with a first‑level adult nursing degree.
• Current registration with the Nursing and Midwifery Council (NMC).
• Experience in community or district nursing, preferably at Band 5 level.
• Commitment to ongoing professional development and lifelong learning.

Required skills

• Basic life support (BLS) skills.
• Use of clinical risk‑assessment tools.
• Implementation of evidence‑based nursing interventions.
