Community Staff Nurse – District Nursing (Part‑time)

About the role

This part‑time permanent position is based at Possilpark Health & Care Centre and offers a rewarding opportunity to deliver high‑quality, person‑centred nursing care to patients in their homes and community settings across Greater Glasgow.

Key responsibilities

• Assess, plan, implement and evaluate care programmes in partnership with patients, families and carers.
• Provide skilled nursing interventions for individuals with complex and long‑term conditions.
• Manage and prioritise workload to meet both planned and unplanned care needs.
• Maintain accurate, up‑to‑date clinical records in line with professional standards.
• Offer clinical leadership and act as a role model for junior staff and students.
• Collaborate with multidisciplinary and multi‑agency teams to coordinate care and support discharge planning.
• Promote patient safety through risk assessment, incident reporting and adherence to policies.

Required profile

• Registered Nurse with a current NMC UK PIN.
• Relevant post‑registration experience in community or district nursing.
• Evidence of continuing professional development and a commitment to lifelong learning.
• Ability to work independently and autonomously in a community setting.
• Strong communication, interpersonal and organisational skills.
• Experience delivering person‑centred care to patients with complex needs.
