Community Support Worker – Mental Health for Older People

About the role

The Community Support Worker provides mental health care and support to older people living in the community, working under the direction of qualified nurses and social workers. The role involves both independent work and close collaboration with a multi‑disciplinary team to deliver personalised care plans.

Key responsibilities

• Appraise senior staff on service users' progress and document changes in mental health condition.
• Assist in implementing care plans while safeguarding users' privacy, dignity, choice and independence.
• Participate in multi‑professional assessments, reviews and Care Programme Approach meetings.
• Provide practical and emotional support to service users and carers, including maintaining social networks and liaising with other agencies.
• Report any changes in circumstances to relevant agencies to ensure continuity of care.
• Promote non‑discriminatory care and develop awareness of cultural and ethnic needs.

Required profile

• GCSE grade C or above (or equivalent) in English and Maths.
• Computer literate – able to use Microsoft Word, Excel and PowerPoint.
• Knowledge of mental health and social care legislation (e.g., Mental Health Act, Mental Capacity Act).
• Ability to manage distressing and challenging situations.
• Excellent communication skills, both verbal and written.
• Valid car driver’s licence (unless exempt under the Equality Act 2010).
