About the role
As a Complex Care Coordinator you will help clients live safely and with dignity by managing rosters, matching healthcare assistants and nurses to care packages, and acting as the main point of contact for families and professionals. You will travel across Doncaster and surrounding areas, so a full UK driving licence and your own vehicle are essential.
Key responsibilities
- Provide first‑point client, family and professional support.
- Manage rosters and make fast, confident scheduling decisions.
- Monitor care delivery to ensure compliance with care plans and quality standards.
- Attend multi‑agency ICB meetings and advocate for client needs.
- Support the out‑of‑hours on‑call service (no hands‑on care).
Required profile
- Previous Care Coordinator experience preferred, or strong administrative background with excellent organisational skills.
- Clear, adaptable communication and calm, solution‑focused decision‑making.
- Understanding of person‑centred care and care planning.
- Confident negotiation, influencing and relationship‑building abilities.
