Complex Care Nurse Consultant – Home-Based

About the role

We are seeking experienced Complex Care Nurse Consultants to join our Community & Complex Care at Home team. In this role you will provide high‑quality, person‑centred clinical care to individuals with complex health needs in their own homes, supporting them to live safely and independently.

Key responsibilities

• Deliver and oversee complex clinical care for service users in their homes.
• Conduct comprehensive clinical assessments and develop person‑centred care plans.
• Provide clinical leadership, supervision and support to care staff.
• Carry out competency assessments and deliver training for support workers.
• Monitor and review care packages to ensure safe, effective service delivery.
• Collaborate with families, multidisciplinary teams and other healthcare professionals.
• Ensure compliance with clinical governance, safeguarding and regulatory requirements.
• Maintain accurate clinical records and documentation.

Required profile

• Current NMC registration (Adult, Children’s or Learning Disabilities Nurse).
• Previous experience supporting individuals with complex healthcare needs.
• Strong leadership, communication and organisational abilities.
• Full UK driving licence and access to a vehicle (desirable).
