Customer Service Coordinator – Medical (Hospital to Home)

About the role

Join a leading healthcare provider to be the first point of contact for patients transitioning from hospital to home. As a Customer Service Coordinator you will guide patients and carers through the onboarding process, ensuring timely delivery of medical supplies and a seamless experience.

Key responsibilities

• Initiate patient onboarding by setting up accounts and confirming healthcare details.
• Coordinate initial deliveries and verify receipt with patients.
• Liaise with patients, carers, GPs and healthcare professionals to maintain service continuity.
• Resolve queries via phone and email, prioritising customer excellence.
• Maintain accurate patient records and update internal databases.
• Participate in a pre‑planned on‑call rota providing out‑of‑hours support with enhanced pay.

Required profile

• Empathetic, patient‑focused attitude with a genuine desire to improve lives.
• Professional telephone manner and clear written communication.
• Reliability and flexibility to work rotating shift patterns, including weekends.
• Previous experience in a busy customer service environment.

Required skills

• Proficiency with database navigation and rapid adoption of new IT systems.
