Discharge Link Nurse – Home First Team (Secondment)

About the role

This fixed‑term secondment position (9 months) is for an experienced registered nurse to join the Home First Discharge Team within the Betsi Cadwaladr University Health Board. Based primarily at Ysbyty Gwynedd, the role supports effective discharge planning across Gwynedd and Mon, helping patients transition safely from hospital to community care.

Key responsibilities

• Provide specialist knowledge and leadership to acute and community staff to promote timely discharge.
• Implement the Health Board’s Discharge to Recover & Assess (D2RA) strategy.
• Collaborate with local authority, voluntary sector and multidisciplinary teams to develop care models that enable patients to leave hospital when ready.
• Identify barriers to discharge, lead work streams to address them, and produce reports on findings.
• Deliver education and training programmes for staff on discharge processes.

Required profile

• Registered Nurse with experience in discharge planning or a related specialist area.
• Knowledge of the Discharge Process, Funded Nursing Care, and the Continuing Healthcare & Social Well‑Being (Wales) Act 2014.
• Excellent communication, assessment and documentation abilities.
• Ability to work collaboratively within a multidisciplinary environment.
• Welsh language skills are desirable but not mandatory.

• Strong presentation and training delivery skills.
