About the role
The West Dunbartonshire District Nursing Team is seeking a District Nurse Specialist Practitioner to deliver high‑quality, person‑centred care in patients' homes and the community. Working autonomously, you will lead a caseload, support the nursing team and collaborate with multidisciplinary partners to improve outcomes.
Key responsibilities
- Lead and manage a defined district nursing caseload, ensuring safe, effective, person‑centred care.
- Provide skilled clinical interventions and act as the clinical expert within community nursing practice.
- Coordinate workload, prioritise care needs and respond to both planned and urgent unscheduled requests.
- Work collaboratively with multidisciplinary teams, patients and carers to plan and deliver holistic care.
- Delegate, supervise and support team members to maintain high standards of practice.
- Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards.
- Contribute to service improvement, audit, education and the ongoing development of the district nursing team.
Required profile
- Registered Nurse with current NMC registration in Adult Nursing.
- Specialist Practitioner qualification in District Nursing at Master's level or equivalent.
- V300 Independent Prescribing qualification and advanced clinical assessment skills.
- Demonstrable leadership and management experience within a community or primary‑care setting.
- Evidence of continuous professional development and commitment to lifelong learning.
Required skills
- Independent Prescribing (V300) qualification.
- Proficiency with electronic patient record systems.
