District Nurse Specialist Practitioner – Community Nursing

About the role

An exciting opportunity has arisen within the West Dunbartonshire District Nursing Team for a District Nurse Specialist Practitioner working within NHS Greater Glasgow and Clyde. This rewarding and dynamic role involves delivering high‑quality, person‑centred care to patients in their own homes and communities.

Key responsibilities

• Lead and manage a defined district nursing caseload, ensuring safe, effective, person‑centred care delivery.
• Provide skilled clinical interventions and act as the clinical expert within community nursing practice.
• Coordinate workload, prioritise care needs and respond to both planned and urgent unscheduled requests.
• Work collaboratively with multidisciplinary teams, patients and carers to plan and deliver holistic care.
• Delegate, supervise and support team members to maintain high standards of practice and service delivery.
• Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards.
• Contribute to service improvement, audit, education and the ongoing development of the district nursing team.

Required profile

• Registered Nurse with current NMC registration in Adult Nursing.
• Specialist Practitioner qualification in District Nursing at Master's level or equivalent.
• V300 Independent Prescribing qualification and advanced clinical assessment skills.
• Demonstrable leadership and management experience within a community or primary‑care setting.
• Evidence of continuous professional development and commitment to lifelong learning.
