Extra Care Support Coordinator

About the role

We are looking for a warm, compassionate and proactive Extra Care Support Coordinator to join our team in Harrow. You will help deliver person‑centred support to up to 60 residents, working closely with the Extra Care Registered Manager to ensure a high‑quality, empowering service.

Key responsibilities

• Provide personal care, domestic assistance and support to individuals in line with their care plans.
• Assist residents in accessing work, education, and social opportunities.
• Work flexibly on a roster that includes evenings and occasional weekends.
• Collaborate with families, friends and external agencies to promote a positive atmosphere.
• Support the Registered Manager in day‑to‑day service delivery and quality monitoring.

Required profile

• Previous practitioner experience working with vulnerable adults.
• Flexible, energetic, reliable and able to work a varied roster.
• Gumption, a can‑do attitude and a genuine commitment to person‑centred care.
• Willingness to undertake continuous training and professional development.
