Head of Age Well

About the role

Southwark Council, in partnership with SEL ICS, is seeking a Head of Age Well to lead the NHS Continuing Health Care (CHC) team and Better Care Fund (BCF) initiatives. The role is hybrid, with 2‑3 days a week in the office and flexibility for external meetings.

Key responsibilities

• Manage the CHC team and oversee BCF proposal development and reporting to NHS England.
• Lead a commissioning team focused on older, frail, and physically disabled residents in Southwark.
• Build collaborative relationships across the council, NHS, voluntary sector and Partnership Southwark.
• Develop and deliver a programme of work targeting health and wellbeing inequalities.
• Assess current commissioning arrangements and create a strategic commissioning plan.
• Contribute to professional development of team members and coordinate complex programme delivery.

Required profile

• Significant experience working in the NHS or an integrated commissioning function.
• Proven track record managing CHC teams and BCF initiatives.
• Ability to develop proposals, produce reports for NHS England and influence public‑health commissioning.
• Strong stakeholder engagement skills across council, health system and voluntary partners.
