Head of Age Well

About the role

The Head of Age Well is a senior commissioning position jointly funded by the NHS and Southwark Council. The role requires proven experience within the NHS or an integrated commissioning function, particularly in managing NHS Continuing Health Care teams and Better Care Fund initiatives.

Key responsibilities

• Lead a commissioning team focused on older, frail, and physically disabled residents in Southwark, developing and delivering a targeted programme of work.
• Build and maintain collaborative relationships across Southwark Council, the wider health system, Partnership Southwark, and voluntary sector partners.
• Assess current commissioning arrangements, work with Public Health Consultants and policy officers to create and implement a division‑wide commissioning plan.
• Coordinate delivery of complex programmes addressing health and wellbeing inequalities, ensuring value and improved outcomes for residents.

Required profile

• Demonstrated experience working in the NHS or an integrated commissioning environment.
• Experience managing NHS Continuing Health Care (CHC) teams and Better Care Fund (BCF) projects, including proposal development and reporting to NHS England.
• Ability to engage senior stakeholders such as public health consultants, heads of programme, and policy officers.
• Willingness to attend occasional evening and weekend meetings across the borough.
