Health Care Assistant – Bristol, Bath, Swindon

About the role

As a Health Care Assistant you will provide person‑centred support to residents and service users in private care homes and community settings across Bristol, Bath and Swindon. You will work either alongside another carer or deliver dedicated 1:1 care, ensuring safety, dignity and independence for each individual.

Key responsibilities

• Deliver safe, respectful, person‑centred care in line with individual care plans.
• Assist with personal care tasks such as washing, dressing, toileting and continence support.
• Use moving and handling equipment (hoists, transfer aids, wheelchairs) safely following training and risk assessments.
• Provide dedicated 1:1 care, monitoring residents closely and taking action to reduce risks of falls, wandering or challenging behaviour.
• Offer companionship, meaningful activities and encourage meals and fluid intake.
• Support safe mobility, repositioning and pressure‑area care.
• Prompt or support medication only when trained, authorised and instructed.
• Complete accurate care records, including hourly 1:1 notes, and report any changes in health or safeguarding concerns promptly.
• Communicate professionally with residents, families, staff and healthcare professionals.
• Travel to service users’ homes to provide community‑based personal care, meal preparation, medication prompts, light household duties and companionship.

Required profile

• Commitment to delivering respectful, dignified care that promotes independence.
• Ability to work collaboratively with another carer or independently in a 1:1 setting.
• Observant and responsive to changes in residents’ health, behaviour or wellbeing.
• Willingness to follow care plans, risk assessments and safeguarding procedures.

Required skills

• Operation of moving and handling equipment (hoists, transfer aids, wheelchairs).
• Medication prompting (when trained and authorised).
• Accurate care record documentation.
