Health & Wellbeing Practitioner – HMP Downview

About the role

We are seeking a Health & Wellbeing Practitioner to join The Forward Trust’s integrated team at HMP Downview in Sutton. Working full‑time (37.5 hours per week), you will provide trauma‑informed, gender‑responsive support to women in custody, helping them overcome substance misuse and low‑level mental health challenges and preparing them for a healthier future on release.

Key responsibilities

• Manage a varied caseload and develop personalised recovery plans.
• Conduct assessments and risk assessments to guide treatment pathways.
• Deliver one‑to‑one interventions and facilitate group wellbeing sessions.
• Provide harm‑reduction advice and practical support.
• Support release planning and ensure continuity of care through the gate.
• Collaborate closely with healthcare, mental health and prison staff.
• Maintain accurate case records and contribute to service performance targets.
• Undertake second‑signatory duties and assist clinical colleagues as required.

Required profile

• Demonstrated passion for supporting people to achieve positive change.
• Experience in substance misuse, mental health, criminal‑justice or health‑social‑care environments.
• Strong communication and relationship‑building abilities.
• Ability to manage a busy caseload and prioritise effectively.
• Collaborative, solution‑focused approach with excellent record‑keeping.
