Health & Wellbeing Practitioner – Probation Support

About the role

This permanent, full‑time position supports adult and young men in the Criminal Justice System to improve their health, build resilience and successfully reintegrate into the community. Working alongside experienced practitioners and partner agencies, you will deliver personalised wellbeing interventions across custody and community settings.

Key responsibilities

• Manage a caseload of clients, building trusted relationships and assessing individual needs.
• Develop and deliver tailored one‑to‑one and group sessions covering family relationships, lifestyle, emotional wellbeing and social inclusion.
• Provide practical advice, guidance and wellbeing interventions to promote healthier, more independent lives.
• Collaborate with prison teams, community services and partner agencies to ensure seamless support.
• Record outcomes, monitor progress and contribute to service performance targets.

• Excellent communicator able to build trust with diverse backgrounds.
• Organised, resilient and confident managing a varied caseload.
• Comfortable delivering both individual and group interventions.
• Team‑player who enjoys working with partner agencies.
