About the role

Join the Gravesham Central Community Team at Gravesham Community Hospital as a Healthcare Assistant. You will deliver personalised care in patients' homes and local clinics, helping them stay independent and supported within their community. The role is ideal for someone who enjoys varied, community‑based work rather than being stationed in a single location.

Key responsibilities

• Provide delegated nursing care to patients in their own homes and local clinics.
• Carry out clinical tasks such as dressings and glucose monitoring after training and competency sign‑off.
• Support holistic, evidence‑based care tailored to each patient’s needs.
• Plan and manage your own visit schedule across the Gravesend area.
• Collaborate closely with registered nurses and the wider multidisciplinary team.

Required profile

• Enthusiasm for community‑based healthcare delivery.
• Ability to work independently while maintaining strong communication with the nursing team.
• Willingness to develop clinical skills through training and competency assessment.
• Commitment to delivering compassionate, patient‑focused care.
